Program Change Request Form

Please submit the following form in order to be considered for a streamlined process to transition from a certificate program into an Master of Divinity, MA in Theological Studies, MA in Theology and Ministry, or MA in Global Missional Leadership program.

Please ensure that your answers to these questions are accurate. If it is determined that your answers are not accurate, it may result in delays to your enrollment process and/or a change to your enrollment status. If you have questions about your cumulative Fuller GPA and/or your current enrollment status, please contact your Academic Advisor (advising@fuller.edu).


If you do not hold an accredited bachelor's degree or master's degree, you do not qualify for this streamlined process. Please contact the Admissions Office for reapplication procedures.


If you have not yet satisfied the terms of your probation, or conditional, admission status you do not qualify for this streamlined process. Please contact the Admissions Office for reapplication procedures.

If your Fuller CGPA is below 3.0, you do not qualify for this streamlined process. Please contact the Admissions Office for reapplication procedures.

If you last successfully completed a Fuller class over 4 quarters ago, you do not qualify for this streamlined process. Please contact the Admissions Office for reapplication procedures.

Please note: You must complete your Certificate coursework before the quarter you plan to start the master's level program. If you do not, your admission to the master's program may be deemed invalid and there may be financial aid implications.
